Addiction Assessment Tool

I am presenting a lesson to the teen youth group at our church about addictions. I'm looking for some type of questionnaire they can answer as an assessment tool. I want to ask them about any possible stresses and peer pressure in their lives.



I want to help them identify factors in their lives which may lead them to addictions and give them resources to deal with their emotions in a healthy manner.



Any information you can provide would be greatly appreciated.
